what we do
Yellow House Foundation
and its contracted provider Gateway Support Services, Inc is pleased to announce a new program to young adults between the ages of 18-25 offering the support they need to live the life they desire to have. Moving forward and on their way to self-sufficiency.
Program Guide
The Goal in this stage is to train and educate the young adult about their personal diagnosis/mental health and or disability. To assist the young adult with gaining a grounded foundation. One that is personalized to them.
Goal here is to learn independence, responsibility for self, and to be as responsible as possible. This stage time frame varies 2 years—4 years
The goal is for the young adult to be able to perform stages I & II with no assistance. Paid supports to be available for 6 months- 1 year to offer support as needed.
